No, he's saying that if you are fighting the outer guardian, and leave your mob can't get your item for you. This is the same as if you are in the only cabal member in a group that is killing the outer guardian, you won't get your item unless you are in the room with the guardian when it dies.

Only for the inner guardian. The outer guardian only gives up a cabals item if a cabal member is part of the group that kills it.
