Dload the Mud Client "MUDMAGE" a bit of searching will find it. It is a good MUD Client in it's own, but what makes it nice is it contains a portion for writing notes.

YOu type your note in the windows, hit the send note button and off it goes. Why this is good, is because under general options, you change change the note settings. So change note + to desc + or role + etc. and you get a perfectly formed desc role note whatever EVERY time. I personally type all mine out in MS-Word, then copy the whole damn screen, and paste it into the MUDMAGE windows, it will parse where necessary. No muss no fuss.
